Level 1 Membership is good for 30 days from date of entry.
Level 2 Entry Requirements:
1. Valid Level 1 Member, and
2. Refer 1 Surgery Patient
Receive 10% off regular priced surgery and 20% off retail products, B-12 Lipo Injections, and Juvederm.
Level 2 Membership is good for 6 months from date of surgery.
Level 3 Entry Requirements:
1. Valid Level 2 Member, and
2. Refer 3 or more Surgery Patients within one calendar year
Receive 20% off regular priced surgery and 20% off retail products, B-12 Lipo Injections, and Juvederm.
Level 3 Membership is good for 12 months.
For Surgery Patient Referrals to be vaild, the patient that was referred must 1) put the referring patient's name where requested on the new patient form 2) on the date of their first visit in the office and 3) membership activates on the date the referred patient becomes a qualified "Surgery Patient."
